LMC Pharmacy – Diabetes Source, an affiliate of LMC Diabetes & Endocrinology, is an innovative in-clinic Pharmacy specializing in diabetes and endocrine disorders. Staffed by CDE Pharmacists, LMC Pharmacy – Diabetes Source conveniently provides MedsChecks and an extensive line of medications and supplies.

Currently, our growing team is looking for a CDE Pharmacist (FT) to join us at our LMC Pharmacy in Barrie, Ontario.

Summary:

  • Reporting to the Director of Pharmacy, the Pharmacist will provide diabetes counseling and dispensing services at LMC Pharmacy – Barrie
  • Role provides a unique opportunity to practice in a collaborative clinical environment with a team of top specialists in diabetes and endocrinology
  • The Pharmacist will provide focused care to a rapidly growing base of patients with diabetes and other endocrine conditions

Responsibilities:

  • Working with the Clinic and Diabetes Education teams to provide diabetes counseling and medication reviews for patients via the MedsCheck program
  • Collaborate with endocrinologists and other team members to provide best pharmaceutical care to patients
  • Support all responsibilities related to pharmacy operations as well as medication/supplies dispensing

Requirements:

  • Must be a licensed pharmacist (active Part A registration with the Ontario College of Pharmacists)
  • 4-5 years of Ontario pharmacy practice and diabetes education experience preferred
  • Possessing a Certified Diabetes Educator (CDE) designation
  • OCP Injection certified
  • Must have previous retail pharmacy experience
  • Must have previous experience using Kroll
  • Excellent verbal and written communication skills
  • Excellent interpersonal, organizational and time management skills
  • Regular business hours, with flexibility to provide infrequent after-hour service as needed
